Holisticare Hospice is seeking a Per Diem Hospice Aide (CNA/HHA) to join our compassionate team. You will provide hands-on end-of-life care, ensuring comfort, dignity, and emotional support for patients in the Atlantic, Cape May, and Cumberland County areas.

The role requires a valid HHA or CNA license, HS diploma, and at least 6 months hospice or skilled nursing experience. Schedule includes PRN evenings and weekends with mileage reimbursement and comprehensive benefits offered.

Formación

• High School Diploma or GED is required.
• Valid Certified Home Health Aide (HHA) license OR Certified Nursing Assistant (CNA) license is required.
• Minimum of 6 months hospice or skilled nursing facility experience preferred.
• Current driver’s license, vehicle insurance, and reliable transportation required.

Responsabilidades

• Provide personalized care according to the plan of care for each patient.
• Assist with personal care, bathing, oral hygiene, bedding, clothing, and grooming.
• Deliver exceptional customer service and maintain patient satisfaction.
• Support families with guidance and education for caring for terminally ill loved ones.
• Maintain accurate documentation in patient records and agency systems.
