Job summary

Journey Hospice is seeking a per diem, as needed, CHHA to serve patients in the Atlantic and Cape May County areas. The role includes providing personal care and light housekeeping in the home, facility, or other environments per the established plan of care and under RN supervision.

Qualified candidates will have a high school diploma or GED, completion of a NJ Hospice Aide/Homemaker certification, and the ability to work within a multidisciplinary team.

Qualifications

• Must have a High school diploma or GED.
• Completion of a NJ Hospice Aide/Homemaker certification per applicable regulations.
• Minimum 1 year of direct patient care experience preferred in hospice/home care.

Responsibilities

• Provide personal care and light housekeeping to terminally ill patients as per care plan.
• Collaborate with the Hospice IDT and RN supervisor to coordinate patient care.
• Observe, report, and document patient observations and care performed.
• Maintain timely documentation of services per patient care plan.
