Job summary

Journey Hospice is seeking a per diem, as needed, Certified Home Health Aide (CHHA) to serve patients in the Atlantic and Cape May County areas. The role offers premium pay at $25/hour with a weekend shift differential and requires compassionate, patient-focused care in home or facility settings.

The CHHA will provide personal care, light housekeeping, and supportive services under the supervision of a Registered Nurse, while collaborating with the Hospice IDT to ensure quality patient outcomes.

Qualifications

• High school diploma or GED required.
• CHHA certification from NJ Board of Nursing preferred and proof of certification.

Responsibilities

• Provide personal care and light housekeeping to terminally ill patients in home or care facilities.
• Participate as a member of the Hospice Interdisciplinary Team (IDT).
• Document observations and care provided, ensuring timely updating of patient records.
• Follow the established plan of care under RN supervision.
