Qualifications

• One year of related OT experience; mentoring if less than one year.
• Current CPR certification.

Responsibilities

• Deliver OT services per physician-approved plan of care.
• Ensure the plan addresses problems identified in assessments.
• Ensure safety and effectiveness of OT service delivery.
• Complete documentation timely and accurately.
• Supervise home health aide when RN is not active.
• Perform other duties as assigned.
