Qualifications

• Current license to practice occupational therapy in the state you are assigned to work.
• Graduate of an ACOTE‑accredited OT program.
• Eligible to take or has completed NBCOT certification examination.
• One year related OT experience.
• If less than one year, regional clinical leaders' approval and mentoring/residency may be required.
• Current CPR certification.
• Driver's license and reliable transportation.

Responsibilities

• Evaluate and treat patients in their homes.
• Promote independence, safety, and quality of life.
• Develop individualized treatment plans per physician orders and agency policies.
• Collaborate with patients, families, and interdisciplinary team members.
• Ensure safe, effective OT service delivery under plan of care.
• Complete timely and accurate documentation and supervise aides when needed.
