UnitedHealth Group seeks an Occupational Therapist Per Diem to evaluate and treat patients in their homes, promoting independence and safety. You will collaborate with patients, families, and a multidisciplinary team to develop individualized plans aligned with physician orders and agency policies.

Qualifications

• ACOTE-accredited OT program graduate.
• Current license to practice OT in the assigned state.
• Current CPR certification.
• NBCOT certification examination eligibility.
• 1+ year OT experience.
• Valid driver's license and reliable transportation.

Responsabilités

• Responsible for all aspects of occupational therapy service delivery provided under a physician approved plan of care
• Ensures the plan of care addresses all problems identified in the assessment or documents rationale if identified problems are not addressed in the plan of care
• Accountable for the safety and effectiveness of the occupational therapy service delivery process
• Completes documentation timely and accurately
• Supervises home health aide when RN is not active in the case
• Other duties as assigned
