Job summary

Lehigh Valley Health Network is seeking a Physical Therapist for Home Care on a per diem basis. The role involves evaluating patients, planning and directing PT programs to restore function and independence, and collaborating with families and the care team across multiple locations.

You will implement evidence-based therapy techniques, document outcomes, and adapt plans to each patient’s evolving needs while upholding safety and quality standards.

Qualifications

• Must be able to provide safe, compassionate, and competent physical therapy treatments.
• Ability to travel to patient care assignments and work at multiple locations per diem.
• Excellent communication with patients, families and colleagues.

Responsibilities

• Evaluates and administers PT programs to restore function and independence.
• Develops and updates a plan of care with patient/family.
• Provides prescribed exercise, manual therapy and patient education.
• Documents progress and modifies plans as needed.
• Collaborates with the care team and supervisor.

Education

Bachelor’s Degree Physical Therapy

Master’s Degree Physical Therapy or Doctorate Physical Therapy
