Qualifications

• Bachelor’s Degree in Physical Therapy required and valid licensure.
• Clinical reasoning to synthesize evaluation data and assign diagnoses.
• Ability to travel to patient care assignments for Home Care.
• PT license in the state of Pennsylvania and BLS certification.

Responsibilities

• Establishes and implements PT plans of care to restore function and independence.
• Uses evidence-based techniques such as therapeutic exercise and manual therapy.
• Performs reassessments and updates care plans accordingly.
• Communicates progress to supervisors, physicians, and team members.
• Provides cost-effective care and adheres to billing guidelines.
