Lehigh Valley Health Network is seeking a licensed Physical Therapist to evaluate, plan, and direct therapy programs to restore function and maximize independence. You will develop plans of care with a patient/family and provide education to improve quality of life.

The role involves evidence-based therapies, collaboration with a care team, and adherence to regulatory standards. Strong communication and compassionate care are essential for success in this on-site position at Schuylkill.

Formación

• Bachelor's Degree Physical Therapy
• Demonstrates sound clinical reasoning in synthesizing data gathered in the evaluation and identifying treatment diagnoses.
• Knowledge of physical therapy techniques.
• Must be able to provide safe, compassionate, and competent physical therapy treatments.
• Must be able to communicate with patients, family, and co-workers in a friendly, supportive manner.
• Ability to travel to patient care assignments - Home Care only
• American Heart Association Basic Life Support - State of Pennsylvania
• Upon Hire PT - Licensed Physical Therapist - State of Pennsylvania
• Master's Degree Physical Therapy or Doctorate Physical Therapy
• 1 year clinical experience for Home Care

Responsabilidades

• Establishes and implements appropriate physical therapy plans of care to restore function, prevent disability, and help patients reach their maximum performance level.
• Implements an individualized treatment plan using evidence-based therapy techniques such as exercises and manual therapy.
• Provides physical therapy treatment consistent with the patients' needs assessment.
• Performs reassessment when indicated and modifies plans accordingly.
• Communicates patient progress to supervisor, physician(s), and team.
• Establishes a physical therapy plan with measurable goals and time frames.
• Provides cost effective care in accordance with billing guidelines.
• Completes and submits all documentation per policies and regulatory requirements.
