Job summary

Lehigh Valley Health Network is seeking a Physical Therapist (PT) for Home Care in the Pocono region. This per diem role requires evaluating and planning PT programs to restore function and independence, with patient education and hands-on care.

Candidates should hold a Bachelor’s in Physical Therapy and be able to travel to patient care assignments. Master’s or Doctorate in PT is preferred. Washington safety and state licensure requirements apply.

Qualifications

• Bachelor’s Degree in Physical Therapy is required.
• Ability to travel to patient care assignments – Home Care.
• Knowledge of best practices in physical therapy and clinical reasoning to determine treatment diagnoses.

Responsibilities

• Establishes and implements appropriate physical therapy plans of care to restore function and prevent disability.
• Implements individualized treatment plans using therapeutic exercises, manual techniques, mobility training, ADL training, and modalities.
• Provides physical therapy treatment aligned with patient needs assessment and plan of care.
• Performs reassessment as needed and updates plans.
• Communicates patient progress and limitations to supervisor, physicians, and team members.
• Provides care with measurable goals and time frames and ensures documentation complies with policies.
