Qualifications

• Bachelor’s Degree in Physical Therapy.
• Per diem location availability.
• Knowledge of best practices in physical therapy.
• Sound clinical reasoning in evaluation data.
• Safe, compassionate, and competent physical therapy treatments.
• Effective communication with patients, families, and staff.
• Willingness to travel to Home Care assignments.
• AHA BLS - State of Pennsylvania Upon Hire.
• PT - Licensed Physical Therapist - State of Pennsylvania Upon Hire.

Responsibilities

• Establish and implement PT plans of care to restore function.
• Use evidence-based techniques such as therapeutic exercises and manual therapy.
• Provides PT treatment based on needs assessment.
• Performs reassessment and adjusts plans accordingly.
• Communicates patient progress to supervisor, physicians, and team.
• Implements plan of care with age-specific needs.
• Provides supervision as required by regulations.
• Establishes treatment plans with measurable goals.
• Ensures cost effective care within billing guidelines.
• Completes and documents per policies and standards.
