Oak Hill seeks a Direct Support Professional (Substitute) in Granby, CT to support individuals in daily living, health, and community activities on a per diem basis. Per diem DSPs work as needed, with a minimum of 15 hours per month; candidates must be 18+, have a high school diploma or GED, and pass background checks.

Qualifications

• Must be at least 18 years of age.
• High school diploma or GED.
• Valid driver's license to transport individuals in agency vehicles.
• Ability to successfully complete required background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain necessary certifications.
• Commitment to maintaining a safe, healthy, and professional work environment in accordance with Oak Hill policies.
• Commitment to continuity of care, including an occasional need to stay beyond a scheduled shift until coverage is in place.

Responsibilities

• Support individuals in daily living activities.
• Monitor health information and vital signs when required.
• Administer medications and maintain medication records as directed.
• Transport individuals to appointments, activities, and events.
• Prepare meals and assist with mealtime.
• Assist with mobility and transfers using approved techniques and equipment.
• Document daily activities and health information in the EHR.
• Follow emergency procedures and safety protocols.
• Collaborate with team members and supervisors.
