You’ll provide person-centered care in residential settings, support daily living, wellness, nutrition, and community participation, and collaborate with a dedicated team.

Required are 18+, HS diploma/GED, valid driver’s license, and ability to pass background checks.

Qualifications

• Must be at least 18 years old.
• High school diploma or GED.
• Valid driver’s license with the ability to transport individuals in agency vehicles.

Responsibilities

• Assist with daily living activities including personal care, grooming, hygiene, and mobility.
• Monitor health and wellness, document information, and assist with medications per training and procedures.
• Support community inclusion and transportation to appointments and activities.
