Endeavor Health is hiring a Social Worker-Discharge Planner in Elmhurst, IL for a per diem schedule. The role involves counseling, discharge planning, and coordinating post-discharge care in collaboration with the healthcare team.

The successful candidate will have a Master’s in Social Work, 1 year of medical social work experience, and an IL LSW license (or license pending with an 8‑month deadline). Per diem hours are Monday–Friday, 8:30am–5:00pm.

Qualifications

• Master's Degree in Social Work is required.
• Minimum 1 year of medical social work experience in a healthcare setting.
• 1 year of clinical rotation in an acute care hospital setting is required.
• 1 year of prior hospital social work experience is required.
• If licensed pending, must obtain LSW license within eight months of hire.

Responsibilities

• Provide comprehensive discharge planning services for identified cases.
• Assess patients' emotional status, psychosocial stability, and family support.
• Coordinate home care referrals, durable medical equipment, and placement needs.
• Maintain accurate medical records including discharge plans and progress notes.
• Communicate with patients and families to facilitate discharge and post-discharge care.

Education

Master's Degree Social Work

1 Year medical social work experience

1 Year clinical rotation in acute care hospital

1 Year hospital social work experience

Licensed Social Worker (LSW) in IDFPR (IL) if license pending
