Job summary

NYU Langone Health is seeking a Per diem Social Worker to join our team for one shift per week. Under supervision, you will provide psychosocial assessments, counseling, and discharge planning to patients assigned by the Department of Social Work.

You will participate in interdisciplinary planning, document activities, and coordinate with Care Management to support timely post-hospital planning. Qualified candidates will have a MSW and NY licensure eligibility.

Qualifications

• Master's Degree in social work from CSWE-accredited program.
• Eligible for LMSW/LCSW or NYS permit per NYS requirements.

Responsibilities

• Provides psychosocial assessment and counseling services, including discharge planning.
• Participates in interdisciplinary planning and meetings as requested.
• Coordinates with care management to ensure effective throughput.
• Documents activities in a timely manner per departmental protocol.
• Supervises social work interns when qualified.
