Job summary

Community First Medical Center in Chicago, IL seeks a full-time Social Worker to support Case Management and Utilization Management, including Behavioral Health. The role requires LSW with LCSW preferred, 2+ years of clinical social work and acute care experience, and familiarity with EMR/EPIC.

8:00 AM to 4:30 PM schedule with full-time benefits. RESPONSIBILITIES include identifying alternative care pathways, evaluating patient status, coordinating discharge planning, and documenting care plans

Qualifications

• LSW required; LCSW preferred.
• 2+ years of clinical social work experience required.
• Acute care social work experience preferred.
• Bachelor’s degree required.
• EMR/EPIC experience preferred.

Responsibilities

• Identify alternative practice among physicians, nurses and other care providers.
• Evaluate and document patient status and post-hospital needs.
• Monitor delivery of care and patient response during hospitalization.
• Assess patient status to revise care plans.
• Discuss treatment plans with patient and family.
