Job summary

Johnson Memorial Hospital is seeking an LMSW Social Worker / Discharge Planner to join our Case Management team. The social worker will assess patients and provide direct social work services from admission through discharge.

You will perform ongoing case management, develop discharge care plans, communicate effectively with staff, patients and families, and use standard social work interventions while acting as a resource to other staff.

Qualifications

• MSW degree from an accredited School of Social Work required.
• CT State Social Work Licensure LMSW preferred.
• Certification in CCM and ACCC preferred.
• Knowledge of Medicare, Medicaid, and managed care regulations and trends.
• Experience with utilization management, home care, community resources and rehabilitation beneficial.

Responsibilities

• Provide ongoing case management support to the patients.
• Assesses patient's needs and develops discharge care plans.
• Communicate effectively with staff, colleagues, patients, and family members.
• Utilizes standard Social Work intervention techniques collaborating with patients and families.
• Work as a resource to other staff members.
