Job summary

Swedish Hospital in Elmhurst, IL seeks a Social Worker-Discharge Planner for a per diem role. You will assess patients, coordinate post-acute care, and support families in discharge decisions. Travel to other Endeavor locations may be required.

Responsibilities include comprehensive discharge planning, psychosocial assessment, and collaboration with the care team. Requires MSW, hospital social work experience, and IL LSW eligibility with licensure within eight months.

Qualifications

• Master's Degree in Social Work required.
• 1 year of medical social work experience in a health care setting.
• 1 year of a clinical rotation in an acute care hospital setting.
• 1 year of previous hospital social work experience.
• Licensed Social Worker (LSW) in Illinois or must obtain license within eight months of hire.

Responsibilities

• Provide comprehensive discharge planning and counseling for identified patients and families.
• Assess emotional status, psychosocial stability, and support systems.
• Coordinate home care referrals, equipment needs, and placement options.
• Communicate discharge plans to patients, families and staff throughout hospitalization.
• Maintain accurate medical records including assessments and discharge dispositions.

Education

Master's Degree in Social Work

Licensed Social Worker (LSW) – IL IDFPR (or eligible to obtain within 8 months)
