Job summary

UChicago Medicine AdventHealth is seeking a qualified social worker to assess discharge planning needs for inpatients, observation and/or emergency department patients. You will develop discharge plans, coordinate post-acute services, and communicate with payors to secure authorizations as needed.

The role requires a Master’s in Social Work, 2+ years of social work experience, and either an LCSW or LMHC licensure. This is a full-time, day-shift, on-site position located in Hinsdale, IL.

Qualifications

• Master's degree in Social Work is required.
• 2+ social work experience is required.

Responsibilities

• Assesses patients’ and families’ discharge planning needs.
• Develops discharge plans and coordinates post-acute care services.
• Communicates with Payors for authorization for post-acute care as needed.
• Participates in multi-disciplinary rounds to review patient status and discharge plans.
