Job summary

Endeavor Health in Elmhurst, IL is seeking a Social Worker-Discharge Planner for a per diem role. You will assess patient needs, plan discharge, coordinate follow-up care, and work with the healthcare team.

Responsibilities include psychosocial assessment, coordinating home care, equipment, and placements; maintaining records; attending meetings; and supporting patients and families through transitions in collaboration with interdisciplinary teams.

Qualifications

• Masters Degree in Social Work required.
• 1 year of medical social work experience in a healthcare setting.
• 1 year of a clinical rotation in an acute care hospital setting.
• 1 year of previous hospital social work experience.
• Licensed Social Worker (LSW) – IDFPR license within eight months of hire.

Responsibilities

• Demonstrates the knowledge and skills to provide age-appropriate care.
• Provides comprehensive discharge planning for identified cases, assessing emotional status and family support.
• Conducts post-acute care and discharge planning activities, coordinating home care referrals, equipment, and placements.
• Screens assigned unit cases based on criteria and pursues identified needs for patients and families.
• Provides psychosocial therapies to assist patients and families in coping with illness or disability.
• Attends patient-centered conferences and care rounds to discuss key social and discharge planning factors.
• Maintains knowledge of current trends in the field by reading literature and attending seminars.
• Maintains accurate medical records including assessments, progress notes, and discharge dispositions.
• Attends required meetings and stays abreast of resources to aid post-discharge continuity of care.
