Qualifications

• MSW degree from an accredited school of Social Work and field experience within a hospital setting.
• Licensed Social Worker (MSW) with NASW code of ethics knowledge required.
• Experience in discharge planning, patient advocacy, and counseling.

Responsibilities

• Assess patients for discharge planning needs.
• Complete initial evaluation and place in medical record per policy.
• Collaborate with Physicians and care teams to plan continuation of care after discharge.
• Coordinate with case managers, HMO/PPO, Nursing and other staff for discharge resources.
• Develop and document the discharge plan in the medical record per policy.
• Act as community liaison to arrange resources and ensure timely discharges.
• Organize patient care conferences to identify discharge needs and educate families.
