Job summary

Molina Healthcare, Inc. is seeking a care coordination professional to support behavioral health management across the continuum. You will collaborate with a multidisciplinary team to ensure member progress toward outcomes and contribute to strategies for cost-effective, high-quality care.

Typical duties include assessments, care planning, and ongoing monitoring with flexible travel up to 40% based on state contracts. Strong communication and clinical platform skills are essential for success.

Qualifications

• At least 2 years health care experience, preferably in behavioral health.
• Licensed behavioral health clinician with active state license or equivalent.
• Valid and unrestricted driver’s license and reliable transportation for travel requirements.
• Experience with persons with severe mental health concerns and substance use disorders.
• Knowledge of whole person care principles and discharge planning.
• Proficient data entry and use of clinical platforms.
• Excellent verbal and written communication skills.
• Proficiency with Microsoft Office suite.

Responsibilities

• Completes comprehensive behavioral health assessments within regulated timelines.
• Develops and implements care plans with member, family, and providers.
• Performs telephonic, in-person, or home visits as required.
• Monitors and documents progress toward goals and adjusts care as needed.
• Maintains ongoing member caseload with regular outreach and management.
• Promotes integration of behavioral health, LTSS, and home/community resources.
• Facilitates care team meetings and interdisciplinary collaboration.
• Uses motivational interviewing and Molina guides to support change.
• Assesses barriers to care and provides coordination and assistance.
