Job summary

Angels of Care Pediatric Home Health is seeking RN and LVN nurses for our pediatric home health team in Spring, TX. You will provide 1:1 skilled nursing at patients' homes, support hospital-to-home transitions, and educate families to ensure safe, continuous care.

The role requires eligibility for Texas RN/LVN license, CPR, and experience with high-acuity pediatric patients. Flexible 12-hour shifts and local travel offered.

Qualifications

• Active Texas RN or LVN license.
• At least 1 year post-license experience.
• Current CPR certification.
• Ability to lift and assist pediatric patients as needed.
• At least 3 years of trach care, ventilator management, and TPN post-graduation.
• Emergency response management experience.
• Pediatric home health experience preferred.
• High-acuity pediatric patients experience preferred.

Responsibilities

• Provide skilled pediatric nursing 1:1 care in patients' homes.
• Perform patient assessments and ongoing evaluations; administer medications, treatments, and therapies.
• Support hospital-to-home transitions for high-acuity patients.
• Educate and support parents, families, and caregivers.
• Assist with discharge support and care coordination.
• Train and support permanent field nurses during transitions.
• Provide first-shift and interim coverage as needed.
• Collaborate with clinical care managers and interdisciplinary teams.
• Respond appropriately to emergencies and changes in patient condition.
