Job summary

Angels of Care Pediatric Home Health is seeking Registered Nurses (RN) and Licensed Vocational Nurses (LVN) for our Pediatric Transitional Nurse Team. You will provide 1:1 skilled pediatric home care, manage high-acuity cases including trach/vent/TPN, educate families, and coordinate with care teams.

This role supports transitions from hospital to home and offers flexible scheduling. We value experienced nurses with CPR certification and the ability to work within a dynamic home-care setting.

Qualifications

• Active Texas RN or LVN license and CPR certification.
• 3+ years of Trach care, Ventilator management, and TPN post-graduation.
• Home Health Care environment experience with high-acuity pediatric patients.

Responsibilities

• Provide skilled pediatric nursing 1:1 care in patients' homes.
• Perform patient assessments and ongoing evaluations; administer medications, treatments, and therapies.
• Support hospital-to-home transitions for high-acuity patients.
• Educate and support parents, families, and caregivers.
• Assist with discharge support and care coordination.
• Train and support permanent field nurses during transitions.
• Provide first-shift and interim coverage as needed.
• Collaborate with Clinical Care Managers and interdisciplinary teams.
• Respond appropriately in emergency situations and changes in patient condition.
