Angels of Care Pediatric Home Health is seeking Registered Nurses (RN) and Licensed Vocational Nurses (LVN) for our Pediatric Transitional Nurse Team in Seagoville, TX. This role focuses on high-acuity pediatric care at home, including trach, vent, and G-tube management, training families, and ensuring safe continuity of care.

Qualifications

• Active Texas RN or LVN/LPN license
• Current CPR certification
• Ability to lift and assist pediatric patients
• Experience with Trach care and Ventilator management
• High‑acuity pediatric patients (preferred)
• Emergency response management

Responsibilities

• Provide skilled pediatric nursing 1:1 care in patients’ homes
• Perform patient assessments and ongoing evaluations
• Administer medications, treatments, and therapies
• Support hospital‑to‑home transitions for high‑acuity patients
• Educate and support parents, families, and caregivers
• Assist with discharge support and care coordination
• Train and support permanent field nurses during transitions
• Provide first‑shift and interim coverage as needed
• Collaborate with Clinical Care Managers and interdisciplinary teams
• Respond appropriately in emergency situations and changes in patient condition
