Angels of Care Pediatric Home Health in Houston, TX is seeking Registered Nurses (RN) and Licensed Vocational Nurses (LVN) to join our Pediatric Transitional Nurse Team. You will support medically fragile children at home, manage high-acuity cases, and train families and caregivers.

This role requires 12-hour shifts with travel within 30–40 miles, 5 days a week, and on-call coverage. We offer a comprehensive benefits package and a values-driven team environment.

Qualifications

• Active Texas RN or LVN/LPN license
• Current CPR certification
• Ability to lift and assist pediatric patients
• At least 3 years of experience with Trach care, Ventilator management, and TPN
• Home Health Care environment experience
• High-acuity pediatric patients
• Emergency response management

Responsibilities

• Provide skilled pediatric nursing 1:1 care in patients’ homes
• Perform patient assessments and ongoing evaluations; administer medications, treatments, and therapies
• Support hospital-to-home transitions for high-acuity patients
• Educate and support parents, families, and caregivers
• Assist with discharge support and care coordination
• Train and support permanent field nurses during transitions
• Provide first-shift and interim coverage as needed
• Collaborate with Clinical Care Managers and interdisciplinary teams
• Respond appropriately in emergency situations and changes in patient condition
