Job summary

Angels of Care Pediatric Home Health is hiring Registered Nurses (RN) and Licensed Vocational Nurses (LVN/LPN) for our Pediatric Transitional Nurse Team to support medically fragile children and families during home care transitions. Roles involve high-acuity care, trach/vent/G-tube management, and caregiver training in home settings.

Qualifications

• Active Texas RN or LVN/LPN license required.
• Current CPR certification required.
• Ability to lift and assist pediatric patients as needed.
• Experience with Trach care and Ventilator management preferred.

Responsibilities

• Provide skilled pediatric nursing 1:1 care in patients’ homes.
• Administer medications, treatments, and therapies.
• Support hospital-to-home transitions for high-acuity patients.
• Educate and support parents, families, and caregivers.
• Assist with discharge support and care coordination.
• Train and support permanent field nurses during transitions.
• Provide first-shift and interim coverage as needed.
• Collaborate with Clinical Care Managers and interdisciplinary teams.
• Respond appropriately in emergencies and changes in patient condition.
