Job summary

Angels of Care Pediatric Home Health, based in Conroe, TX, is hiring Registered Nurses (RN) and Licensed Vocational Nurses (LVN) to support medically fragile children in home settings. You will provide skilled pediatric nursing 1:1, assess and treat, and help families transition from hospital to home care.

Ideal candidates have experience with high-acuity patients, trach, vent, G-tube, and emergency management, and a passion to educate families and coordinate care.

Qualifications

• Active Texas RN or LVN/LPN license
• At least 1 year post-license experience
• Current CPR certification
• Ability to lift and assist pediatric patients as needed
• At least 3 years of Trach care, Ventilator management, and TPN post-graduation
• Emergency response experience in pediatric settings
• Pediatric home health experience
• Preference for high-acuity pediatric patients

Responsibilities

• Provide skilled pediatric nursing 1:1 care in patients’ homes
• Perform patient assessments and ongoing evaluations; administer medications, treatments, and therapies
• Support hospital-to-home transitions for high-acuity patients
• Educate and support parents, families, and caregivers
• Assist with discharge planning and care coordination
• Train and support permanent field nurses during transitions
• Provide first-shift and interim coverage as needed
• Collaborate with Clinical Care Managers and interdisciplinary teams
• Respond appropriately to emergencies and changes in patient condition
