Angels of Care Pediatric Home Health is seeking a Pediatric Transitional Nurse to provide 1:1 skilled nursing in patients' homes for medically fragile children. The role emphasizes safe home transitions, caregiver training, and continuity of care in the home environment.

Required Texas RN/LVN license, CPR, and 1+ year post-license experience, plus 3+ years with Trach, vent, and TPN. Local travel and 12-hour shifts are expected in Pasadena, TX.

Competenze

• Active Texas RN or LVN/LPN license.
• At least 1 year post-license experience.
• 3+ years with Trach care, Vent management, and TPN.
• Pediatric home health experience and high-acuity care.
• Current CPR certification.

Mansioni

• Provide skilled pediatric nursing 1:1 care in patients’ homes.
• Perform patient assessments and ongoing evaluations.
• Administer medications, treatments, and therapies.
• Support hospital-to-home transitions for high-acuity patients.
• Educate and support parents, families, and caregivers.
• Assist with discharge support and care coordination.
• Train and support permanent field nurses during transitions.
• Provide first-shift and interim coverage as needed.
• Collaborate with Clinical Care Managers and interdisciplinary teams.
• Respond appropriately in emergency situations and changes in patient condition.
