Job summary
Angels of Care Pediatric Home Health is seeking Registered Nurses (RN) and Licensed Vocational Nurses (LVN) to join our Pediatric Transitional Nurse Team, delivering 1:1 skilled care in patients' homes. Ideal for nurses experienced with high-acuity pediatric patients, trach, vent, G-tube, and emergency management; you will train families, coordinate care, and support safe transitions from hospital to home.

Qualifications
- Active Texas RN or LVN/LPN license.
- Current CPR certification.
- 3+ years of Trach care, Ventilator management, and TPN experience.
- Home Health Care environment experience.
- Experience with high-acuity pediatric patients.
- Emergency response management.
Responsibilities
- Provide skilled pediatric 1:1 nursing care in patients' homes.
- Perform patient assessments and ongoing evaluations; administer medications and therapies.
- Support hospital-to-home transitions for high-acuity patients.
- Educate and support parents, families, and caregivers.
- Assist with discharge planning and care coordination.
- Train and support permanent field nurses during transitions.
- Provide first-shift and interim coverage as needed.
- Collaborate with Clinical Care Managers and interdisciplinary teams.
- Respond to emergencies and changes in patient condition.
