Angels of Care Pediatric Home Health is seeking Registered Nurses (RN) and Licensed Vocational Nurses (LVN/LPN) for our Pediatric Transitional Nurse Team in Fort Worth, TX. The role focuses on high-acuity pediatric patients, including trach, vent, and G-tube care, with emphasis on caregiver training and safe home-care continuity.

Qualifications

• Active Texas Registered Nurse (RN) or Licensed Vocational Nurse (LVN/LPN) license.
• Current CPR certification.
• Ability to lift and assist pediatric patients.
• Experience with Trach care and Ventilator management.
• Preference for high-acuity pediatric patients.
• Emergency response management experience.

Responsibilities

• Provide skilled pediatric nursing 1:1 care in patients’ homes.
• Perform patient assessments and ongoing evaluations; administer medications and therapies.
• Support hospital-to-home transitions for high-acuity patients.
• Educate and support parents, families, and caregivers.
• Assist with discharge support and care coordination.
• Train and support permanent field nurses during transitions.
• Provide first-shift and interim coverage as needed.
• Collaborate with Clinical Care Managers and interdisciplinary teams.
• Respond appropriately in emergencies and condition changes.
