Job summary

Angels of Care Pediatric Home Health in Pasadena, TX is hiring Registered Nurses (RN) and Licensed Vocational Nurses (LVN) for our Pediatric Transitional Nurse Team to support medically fragile children during transitions to home care.

This role suits nurses experienced with high-acuity patients and skilled in trach, vent, G-tube, and emergency management. We provide comprehensive training, flexible scheduling, and a strong community-focused culture.

Qualifications

• Active Texas RN or LVN/LPN license.
• At least 1 year post-license certification.
• Current CPR certification.
• Ability to lift and assist pediatric patients.
• 3+ years Trach care, Ventilator management, and TPN experience.
• Emergency response management.
• Pediatric home health experience with high-acuity patients.

Responsibilities

• Provide skilled pediatric nursing 1:1 care in patients’ homes
• Perform patient assessments and ongoing evaluations; administer medications, treatments, and therapies
• Support hospital-to-home transitions for high-acuity patients
• Educate and support parents, families, and caregivers
• Assist with discharge support and care coordination
• Train and support permanent field nurses during transitions
• Provide first-shift and interim coverage as needed
• Collaborate with Clinical Care Managers and interdisciplinary teams
• Respond appropriately in emergency situations and changes in patient condition
