Job summary

Angels of Care Pediatric Home Health is hiring Registered Nurses (RN) and Licensed Practical Nurses (LPN) for our Pediatric Transitional Nurse Team to support medically fragile children and their families during critical transitions to home care.

This role focuses on high-acuity patients and requires competency in trach care, vent management, G-tube, and emergency management, ensuring safe, successful discharges and continuity of care until permanent staffing is in place.

Qualifications

• Active, unrestricted RN or LVN/LPN license in Florida
• Current CPR certification
• Ability to lift and assist pediatric patients
• Pediatric home health experience
• Experience with Trach care, Ventilator management, and TPN
• Care for high-acuity pediatric patients

Responsibilities

• Provide skilled pediatric nursing 1:1 care in patients’ homes
• Perform patient assessments and ongoing evaluations
• Administer medications, treatments, and therapies
• Support hospital-to-home transitions for high-acuity patients
• Educate and support parents, families, and caregivers
• Assist with discharge support and care coordination
• Train and support permanent field nurses during transitions
• Provide first-shift and interim coverage as needed
• Collaborate with Clinical Care Managers and interdisciplinary teams
• Respond appropriately in emergency situations and changes in patient condition
