Job summary

Angels of Care Pediatric Home Health is hiring Registered Nurses (RN) and Licensed Practical Nurses (LPN) for our Pediatric Transitional Nurse Team to support medically fragile children and their families during critical transitions to home care.

This role requires confidence with high-acuity patients, trach care, and ventilator management to ensure safe discharges, prevent readmissions, and provide interim coverage until permanent staffing is in place.

Qualifications

• Active, unrestricted RN or LVN/LPN license (PA)
• Current CPR certification
• Ability to lift and assist pediatric patients
• Pediatric home health experience
• Experience with Trach care and Ventilator management
• Care for high-acuity pediatric patients

Responsibilities

• Provide skilled pediatric nursing 1:1 care in patients' homes
• Perform patient assessments and ongoing evaluations; administer medications, treatments and therapies
• Support hospital-to-home transitions for high-acuity patients
• Educate and support parents, families and caregivers
• Assist with discharge support and care coordination
• Train and support permanent field nurses during transitions
• Provide first-shift and interim coverage as needed
• Collaborate with Clinical Care Managers and interdisciplinary teams
• Respond appropriately in emergencies and changes in patient condition
