Job summary

NTSOC is a nonprofit focused on delivering client-centered home health nursing services for children and young adults with disabilities in southern Colorado. This Field Nurse Case Manager will oversee admissions, recertifications, and care coordination with the multidisciplinary team, ensuring compliance with policies and state/federal regulations.

The role reports to the Assistant Director of Nursing, provides education to field staff, and requires travel to client homes.

Qualifications

• Graduate of an Accredited Nursing Program required.
• BSN preferred.
• Colorado RN license in good standing with the Dept. of Regulatory Affairs; driver license must be current.

Responsibilities

• Assess patients to determine eligibility and continued eligibility.
• Coordinate care with the patient’s multidisciplinary care team.
• Submit documentation in a timely, complete and accurate manner.
• Implement physician orders in a timely, complete and accurate manner.
• Travel to client homes regularly for visits.
