Job summary

LOGAN Center is seeking a licensed Physical Therapist to partner with Early Learning programs in St. Joseph County, Indiana, delivering evaluations, assessments, and therapy for children birth through age 5.

The role includes home visits, parent training, and coordinating with families, teachers, and community programs to support gross motor development and functional outcomes. The therapist will document results, plan therapy, and ensure payor guidelines are met.

Qualifications

• Provides pediatric physical therapy services.
• Collaborates with families and other staff.
• Communicates results and progress to payors, families, and teams.
• Maintains thorough records and reports as required by payors.

Responsibilities

• Evaluate individuals or groups in various settings.
• Provide therapy to improve gross motor development, strength, ROM, and mobility.
• Collaborate with families to plan home programming and equipment needs.
• Maintain progress notes and prepare discharge summaries and exit plans.
• Participate in case conferences, staff meetings, and trainings.
