Job summary

Reliatus Behavioral Health in the Phoenix metro area seeks an Occupational Therapist to deliver pediatric therapy across Goodyear, Glendale, and Phoenix locations. On-site role with regular travel; you will develop and implement functional treatment plans, collaborate with ABA, Speech, and feeding teams, and support families in real-world settings.

The ideal candidate holds a Master’s or Doctorate in OT, Arizona license, NBCOT, and enjoys interdisciplinary care, continuous learning, and program

Qualifications

• Master’s or Doctorate degree in Occupational Therapy from an accredited program.
• Current Arizona Occupational Therapist license (or ability to obtain prior to start date).
• Current NBCOT certification.
• Willingness to travel between Goodyear, Glendale, and Phoenix locations.

Responsibilities

• Perform occupational therapy evaluations and reassessments.
• Develop and implement individualized treatment plans addressing functional goals.
• Provide direct on-site OT services across designated clinic sites.
• Travel regularly among Goodyear, Glendale, and Phoenix locations.
• Collaborate with caregivers and interdisciplinary teams to generalize skills.
