Qualifications

• Bachelor Degree in Occupational Therapy from an accredited program.
• OTR/L Certification (NBCOT or AOTA).
• MDH licensure in Minnesota and/or Wisconsin.
• Ability to lift 50 pounds as required.
• Successful background checks and professional conduct.

Responsibilities

• Document therapy services accurately and timely per regulations.
• Provide 1:1 screening, evaluation and direct OT therapy.
• Coordinate assessments and treatment plans with the team.
• Deliver therapy following AOTA standards and facility policies.
• Maintain HIPAA privacy and professional ethics at all times.
