Qualifications

• Master of Social Work required.
• 2 years healthcare social work experience preferred.
• Care management experience preferred.
• Experience in assessments, crisis intervention, bereavement counseling, and discharge planning preferred.
• MSW licensure (MI) required.
• Driver's license (MI) required.

Responsibilities

• Conducts psychosocial assessments with patient and family to address needs and plan.
• Coordinate with alternate care sites and community agencies.
• Document assessment and plan in electronic medical record.
• Provide consultation/resources to healthcare team.
• Maintain up-to-date knowledge of community resources.
• Participate in committees/projects as assigned.
• Stay current on applicable federal/state regulations.
• Provide short-term therapeutic support as needed.
