Corewell Health is seeking an experienced Emergency Department Social Worker to provide psychosocial assessments, crisis intervention and discharge coordination for pediatric patients at Helen DeVos Children’s Hospital in Grand Rapids. The role includes cross-training for adult ED coverage and collaboration with an interdisciplinary team to address complex family needs.

Responsibilities include planning post-discharge care, coordinating with community resources, and documenting plans in the EHR.

Qualifications

• Master's Degree in Social Work required.
• 2 years of relevant healthcare-related experience post-MSW.
• MSW license/credential required upon hire in Michigan.
• Care management experience preferred.

Responsibilities

• Collaborate with interdisciplinary team to assist patients and families with complex psychosocial needs.
• Conduct psychosocial assessments and plan discharge from hospital.
• Provide crisis intervention and link to community resources and referrals.
• Coordinate care across pediatric ED and adult ED as needed.
• Maintain knowledge of community resources and participate in committees or projects as assigned.
