Affimned Home Care in Vernon, NJ seeks an experienced LPN to provide 1:1 pediatric nursing in a private duty home setting. You will care for a medically complex patient with a trach, ventilator, and G-J tube, ensuring safety and clinical stability in a 24/7 home care environment.

The role offers flexible scheduling with day/night shifts and long-term case opportunities, requiring active NJ LPN licensure and BLS certification.

Qualifications

• Active New Jersey LPN license required.
• Experience with trach, ventilator and G-J tube care preferred.
• Current BLS certification.
• Authorized to work in the United States.
• Driver's license preferred.
• Physical exam within 12 months.
• PPD or QuantiFERON within 1 year (or chest X-ray within 5 years).
• Proof of flu vaccine or approved exemption.

Responsibilities

• Provide 1:1 pediatric nursing care focused on safety and stability.
• Manage and monitor tracheostomy and ventilator support.
• Perform trach care, suctioning, and airway management as ordered.
• Administer and monitor G-J tube feedings.
• Assess respiratory status and respond to changes in condition.
• Administer medications and treatments per physician orders.
• Maintain accurate clinical documentation.
• Collaborate with family and pediatric clinical team to ensure continuity of care.

Provide meaningful, life-changing 1:1 pediatric nursing care for a medically complex pediatric patient requiring around-the-clock skilled nursing support. Affimned Home Care is seeking a compassionate and experienced Licensed Practical Nurse (LPN) to provide care for a pediatric patient in Vernon, NJ requiring tracheostomy, ventilator, and G-J tube care.

What You'll Do

Pay Rate

LPN: Up to $40/hr

What You Bring

• Active New Jersey LPN License
• Experience with trach, ventilator, and G-J tube care preferred
• Current BLS Certification
• Authorized to work in the United States
• Valid driver's license preferred
• Physical exam within 12 months
