Job summary

Affirmed Home Care, Inc. is seeking Pediatric LPNs for private duty home care across Wayne, Pompton Plains, and Franklin Lakes, NJ.

Provide 1:1 pediatric nursing, monitor and manage tracheostomy and ventilator care, administer G-tube feedings, and coordinate with families and the pediatric team.

Flexible scheduling with weekend, overnight, and evening shifts; opportunities to build a full-time schedule, with pay up to $40/hr.

Qualifications

• Active New Jersey LPN license.
• Experience with trach, ventilator, and G-tube care preferred.
• Current BLS certification.
• Authorized to work in the United States.

Responsibilities

• Provide 1:1 pediatric nursing care in the home.
• Monitor and manage tracheostomy and ventilator support as ordered.
• Administer and monitor G-tube feedings.
• Perform patient assessments and report changes in condition.
• Administer medications and treatments per physician orders.
• Maintain accurate clinical documentation.
• Collaborate with families, physicians, and the pediatric clinical team.

Available Pediatric Cases

Wayne, NJ

• 15-year-old pediatric patient
• Trach and ventilator dependent
• Private duty 1:1 nursing care
• Weekend 16-hour coverage
• Potential to transition into a full 16x7 case

Pompton Plains, NJ

• Pediatric patient requiring trach, ventilator, and G-tube care
• Overnight skilled nursing support
• 11:00 PM – 7:00 AM shifts
• Approximately 40 hours per week available

Franklin Lakes, NJ

• Pediatric patient requiring private duty nursing support
• Transportation and care coordination assistance
• Monday, Tuesday & Wednesday evening coverage
• Sunday: 1:00 PM – 7:00 PM

What You'll Do

• Provide 1:1 pediatric nursing care in the home setting
• Monitor and manage tracheostomy and ventilator support as ordered

Pay Rate

LPN: Up to $40/hr

What You Bring

• Active New Jersey LPN License
• Experience with trach, ventilator, and G-tube care preferred
• Current BLS Certification
• Authorized to work in the United States
• Valid driver's license and reliable transportation preferred
• Physical exam within 12 months
• PPD or QuantiFERON within 1 year (or chest X-ray within 5 years)
• Proof of flu vaccine or approved exemption
