You will serve as a key contact for families, referral sources, healthcare providers, insurance companies, and internal teams, ensuring accurate documentation and timely onboarding according to intake protocols.

Qualifications

• High school diploma or equivalent required.
• Experience in healthcare administration, patient access, or scheduling preferred.
• Excellent communication and organizational skills are essential.
• Strong attention to detail and ability to manage multiple priorities.
• Proficiency with Microsoft Office including Excel, Word, and Outlook.

Responsibilities

• Handle new patient referrals from hospitals and physicians.
• Collect and verify information for new pediatric home health patients.
• Screen inquiries and follow up with families or referral sources to schedule intakes.
• Verify insurance benefits, including Medicaid and private insurance.
• Coordinate with case managers and clinical teams to complete intakes.
• Maintain accurate documentation and follow intake protocols.
• Support onboarding to ensure all paperwork is completed.
