Job summary

Angels of Care Pediatric Home Health in Sherman, TX is seeking an Intake Coordinator to manage new patient referrals, gather information, verify benefits, and coordinate intake scheduling. This on-site role serves as the main contact for families, referral sources, providers, insurers, and internal teams during onboarding.

The ideal candidate has healthcare administration or care coordination experience, strong organization, and excellent communication.

Qualifications

• High school diploma or equivalent is required.
• Prior experience in administrative/healthcare intake roles is preferred.
• Excellent communication, follow-up, and organizational skills are essential.
• Strong attention to detail and ability to manage multiple priorities.
• Proficiency with Microsoft Office including Excel, Word, and Outlook; bilingual in Spanish preferred.

Responsibilities

• Handle new patient referrals from hospitals, doctors, case managers, and other sources.
• Collect and verify information for new pediatric home health patients.
• Screen inquiry calls and schedule intakes with families or referral sources.
• Verify insurance benefits, including Medicaid and private insurers.
• Coordinate with care teams to assign and complete intakes and ensure paperwork is complete.
• Maintain accurate intake documentation and follow established protocols.
• Support onboarding to ensure timely start of care for new clients.
