Qualifications

• Active RN license in state of practice.
• Minimum 2 years healthcare experience, preferably in care management.
• Valid driver's license and reliable transportation for travel requirements.
• Understanding of EMR and HIPAA compliance.
• Knowledge of community resources.
• Ability to be proactive and detail-oriented.
• Ability to work independently in diverse settings.
• Strong verbal and written communication skills.

Responsibilities

• Completes comprehensive assessments of members per regulated timelines and identifies care management eligibility.
• Develops and implements care coordination plan with member and care team.
• Conducts telephonic, home visits or in-person meetings as required.
• Monitors care plan effectiveness and documents interventions.
• Maintains ongoing member caseload and outreach.
• Promotes integration of behavioral health, LTSS, and community resources.
• Facilitates interdisciplinary care team (ICT) meetings.
• Uses motivational interviewing to support behavior change.
• Assesses barriers to care and provides guidance to members.
• May provide consultation and resources to peers as needed.
• Care manager RNs may handle complex cases and medication reconciliation.
