Job summary

Molina Healthcare, Inc. is seeking a dedicated Care Manager RN to support pediatric populations through care coordination across the continuum.

The role requires independent coordination of care, managing a 30–35 member caseload, and performing assessments, visits, and plan monitoring with a focus on positive patient outcomes. The position is hybrid with about 10% local travel in King County and requires prior remote care management experience in clinical settings.

Qualifications

• At least 2 years of health care experience, preferably in care management or related settings.
• Active RN license in the state of practice.
• Ability to travel 25–40% locally as required by contract.
• Experience coordinating care and working with interdisciplinary teams.
• Knowledge of EMR and HIPAA compliance.

Responsibilities

• Complete comprehensive member assessments within regulated timelines and determine eligibility for care management.
• Develop and implement care plans with members, families, physicians and care teams.
• Conduct telephonic, in-person, or home visits as required.
• Monitor progress, document interventions, and adjust plans as needed.
• Maintain a caseload and provide outreach to members to support outcomes.
• Coordinate integration of behavioral health, LTSS, and community resources.
