Qualifications

• Active RN license in the state of practice.
• Minimum 2 years in health care, care management preferred.
• Valid driver’s license and reliable auto insurance for travel.
• Experience with EMR and HIPAA compliance.
• Knowledge of community resources and ability to work across settings.
• Excellent time management and communication skills.
• Ability to work independently with minimal supervision.
• Proficiency with Microsoft Office.

Responsabilités

• Complete comprehensive member assessments per regulations and determine care management eligibility.
• Develop and implement care coordination plan with member and care team.
• Conduct telephonic, in-person, or home visits as required.
• Monitor care plan effectiveness and document intervention progress.
• Maintain ongoing member caseload and outreach.
• Promote integration of behavioral health, LTSS, and home/community resources.
• Facilitate ICT meetings and collaboration.
• Use motivational interviewing to educate and motivate change.
• Identify barriers to care and assist members accordingly.
• May provide consultation or resources to peers.
• Care manager RNs may handle complex cases and medication reconciliation.
• Travel 25-40% as required by state/contract.
