Molina Healthcare, Inc. is seeking a Registered Nurse Care Manager to provide care coordination across the continuum. The role emphasizes assessments, care planning with the member and care team, and navigation of services to improve outcomes while controlling costs.

The position requires a valid RN license, HIPAA/EMR familiarity, strong communication, independent work style, and ability to travel locally up to 40% as needed by state contracts. EOE.

Qualifications

• At least 2 years experience in health care, preferably in care management, or equivalent.
• RN license active and unrestricted in state of practice.
• Valid driver's license and reliable transportation for local travel.
• Understanding of EMR and HIPAA.

Responsabilités

• Completes comprehensive member assessments per timelines and identifies care management eligibility.
• Develops and implements care coordination plans with member, caregiver, physician and team.
• Performs telephonic, in-person, or home visit assessments as required.
• Monitors care plan effectiveness and documents interventions and progress.
• Maintains ongoing member caseload with regular outreach and management.
• Promotes integration of services across behavioral health, LTSS, and home/community resources.
• Facilitates ICT meetings and collaborates with the care team.
• Educates and motivates members using motivational interviewing and clinical guideposts.
• Identifies barriers to care and provides resources to address concerns.
• May provide consultation to peers as needed.
• Care manager RNs may handle complex cases and medication reconciliation.
