Job summary

Ideal candidates hold a Bachelor's or Master's in behavioral health or a related field, with 2+ years in children’s mental health and in case management within Ohio delivery systems.

Qualifications

• 2+ years in behavioral health, social services, or human services.
• 2+ years of experience with personal computers and MS Office.
• 2+ years in children's mental health, child welfare, or related field.
• 2+ years experience in case management or related areas.
• 2+ years familiarity with Ohio delivery systems and local networks.
• Must reside in the West Region of Ohio and be willing to travel up to 50%.
• Reliable transportation and willingness to work beyond core hours.

Responsibilities

• Coordinate and implement care plan activities and monitor progress.
• Collaborate with case managers, Medical Directors, and health programs to overcome barriers.
• Provide coaching and support to empower members to make informed health choices.
• Serve as single point of contact for members and remediate gaps in care.
