Responsibilities

• Coordinate and implement care plan activities and monitor progress.
• Evaluate member needs, eligibility, and tailor care plans.
• Provide coaching and support to empower healthy choices.
• Serve as a single point of contact for members and address gaps in care.
• Document care activities in compliant records.
